MUSIC DEPARTMENT CHANGE OF MAJOR / MINOR FORM INSTRUCTIONS

Students who wish to change their major (or minor) away from Music must initiate the process by filling out the form below. This form will be automatically sent to the Department Chair, who will in turn forward it to the student's primary applied instructor, large ensemble director, academic advisor/counselor, and the Music Department administrative assistant. This form may also be filled out by those who are withdrawing from the University completely.

After completing this form, the student is to obtain the Change of Major form, fill it out accordingly, and obtain signatures in the order requested.

If you have any questions, please ask your academic advisor/counselor or the Music Department administrative assistant.

Students are strongly encouraged to have conversations with their applied instructors, as well as their ensemble directors, regarding their intention to change their major/minor in order to preserve a positive relationship between the student and his/her professor(s).*
Please check any/all reasons that have contributed to your decision. (Some of these reasons may apply to change of major/minor, while others apply to withdraw from the University--check whichever are relevant to your decision)*
This helps the Music Department determine if there are specific areas that need to be addressed in order to improve the department.
Changing a major (or minor) away from Music will nullify all existing Music Department scholarship agreements. (Scholarships will be nullified at the beginning of the next semester once the change of major/minor is complete) *

If you have questions about the departmental policy regarding scholarships, please:

1) Review your original Scholarship Agreement

2) Consult the Music Student Handbook

3) Meet with the primary ensemble director (or coordinator) for your area (Director of Bands, Director of Choral Activities, or Piano Area Coordinator) for further clarification

We are sorry to see you go, but we hope you will continue to perform in our ensembles and/or attend our concerts and events. If there is anything we can ever do for you in the future, please let us know!
